Notice of Conduent Data Breach
Molina Healthcare (“Molina”) recently received notification from Conduent Business Services, LLC (“Conduent”) of a data breach. Conduent, a vendor and business associate of Molina, provides credit balance auditing services for certain Molina providers. The incident involved the protected health information of some Molina members. 1. You should have received your ID card(s) in the mail.
No ID card? No problem! You can download a copy from your member portal account. Visit MyMolina.com and sign up using the last four digits of your Social Security number.
2. Log into your My Molina member portal to set up your account
3. Download the My Molina® mobile app
4. Schedule a visit with your primary care provider (PCP)
