SWH Provider Relations-for providers to submit Serious Reportable Events (SREs) and Provider Preventable Conditions (PPCs) documentation. All required documentation including event summary of the SRE/PPC, full event details (dates, member ID, provider information), root cause analysis and preventability determination, any payment status and claim amounts.
